Diagnosis is straightforward and starts with a simple blood test. A TSH test is the primary screening tool and will catch hypothyroidism in approximately 95% of cases, according to Mayo Clinic. If TSH is elevated, your doctor will typically also check your free T4 level to confirm the diagnosis and assess severity.

Normal TSH: Typically 0.4 to 4.0 mIU/L (though this range can vary slightly by laboratory and patient age).
Subclinical hypothyroidism: TSH is elevated (usually between 4.5 and 10 mIU/L) but free T4 is still in the normal range. You may or may not have symptoms.

For asymptomatic patients with mildly elevated TSH who don't meet criteria for treatment, the standard recommendation is monitoring with repeat labs every 6 to 12 months to watch for progression.
Treatment: What to Expect
The standard treatment for hypothyroidism is levothyroxine (brand names include Synthroid, Levoxyl, and Tirosint), a synthetic form of the T4 hormone your thyroid is underproducing. It's taken as a daily pill, ideally on an empty stomach 30 to 60 minutes before eating, and it replaces exactly what your body is missing.
